One-on-One Coaching with a Certified Diabetes Educator (CDE)
Patients are partnered with their personal CDE coach throughout the program. This personal relationship builds trust, so that patients feel comfortable expressing their adherence challenges. CDEs are also cheerleaders focused on motivating change and celebrating the successes along the way.
A Personalized Plan for Each Patient
From the very first call, coaches begin developing a customized plan to help patients achieve their health goals.
1. Proactive outreach
It's common for a patient to fall off their care plan once they leave the doctor’s office. Fit4D CDE coaches proactively call patients and dig deep to uncover adherence and knowledge barriers and assess if patients are taking their medications, controlling their blood sugar and getting to their routine screenings.
2. Motivational interviewing
Fit4D CDEs are skilled at building relationships with patients virtually. They work with patients to determine what motivates them and encourage them to take a more active role in their health.
3. Expert Diabetes Education
Each patient's plan is unique. Common topics include the impact of diabetes on overall health, tactics to initiate therapy and improve medication adherence, the importance of preventive care, nutrition, exercise, advice to overcome psycho-social barriers, and more.
4. SMART goals
Once they pinpoint the patient's challenges and motivations, CDEs work with patients to set up their own SMART (Specific, Measurable, Achievable, Relevant and Time-Bound) goals. CDEs provide practical solutions for patients so they can incorporate diabetes management into their daily lives.
To reinforce the coaching experience and enhance learning, CDEs provide tools and resources such as videos, webinars and articles, and refer patients to available client resources.
6. Follow Up
Throughout the program, CDEs follow-up with patients to see how they are faring. This keeps patients accountable and helps determine if any goal adjustments need to be made.

We offer coaching in multiple languages including English and Spanish dialects. Coaches understand cultural sensitivities and enhance patient engagement.
With availability including evenings and weekends, our CDEs provide coaching when convenient for patients, adapting to their schedules, lifestyles and life circumstances.
Building relationships via multiple communication methods — phone calls, texts, emails and more —enables patients to access coaching at their convenience.
