Q: What is this program about?

CareFirst has partnered with a company called Fit4D to offer a no-cost program to help you better manage your diabetes. Once enrolled in the program you will receive one-on-one support from a Certified Diabetes Educator (CDE), who specializes in helping people with diabetes better understand and manage their condition. 

Q: Why should I participate?
Participating in this program will provide you with access to a diabetes expert who will answer your questions and help you achieve your goals.  You will also have access to a diabetes management app, Glooko, which syncs to nearly 100% of glucometers, so you can more easily track your blood sugar levels and share that information with your diabetes coach and your doctor. 

Q: How does it work?

  • Enroll immediately by using this online form or by calling 301-242-9803
  • Coaching starts soon after you enroll and you will work with the same diabetes coach one-on-one for three months.
  • Sessions focus on creating a diabetes management plan that's realistic and sustainable
  • If applicable, you will learn how to monitor your blood sugar using the Glooko app.
  • Once the coaching sessions have ended, you will decide with your coach the ideal pathway for long-term success.

Q: How much does this program cost?
This program is covered as part of your CareFirst benefits at no cost to you. Participation in the program is voluntary and confidential. Your decision to participate in this program will not have any impact on your coverage or benefits.

Q: Where do the coaching sessions take place?

The coaching sessions take place over the phone.  You and your diabetes coach will determine a convenient time for your coaching sessions.  Our coaches are available Monday-Saturday.

Q: I met with a Diabetes Educator when I was diagnosed, do I still need this program?

Diabetes is a chronic condition that requires daily maintenance.  Having added support from a diabetes expert to help you meet your short and long-term goals is key in achieving optimum control.  

Q: Will my Diabetes Educator provide me with information about what we talked about?

If you opt-in to email you will have the opportunity to receive information on various topics related to diabetes. This information will be geared towards what you are working on with your Diabetes Coach.  As an added benefit to the program our Diabetes Coaches host monthly webinars (online presentations) that are offered at no cost to you.
