The Fit4D Diabetes Educator INSIDE program provides access to diabetes experts who can deliver effective programs designed to improve clinical health outcomes. Fit4D educators include dietitians, nurses, exercise physiologists, social workers and pharmacists who are able to build trusted and credible relationships with your members. We leverage technology to scale our program's reach as we engage with patients and their healthcare team to keep your members on therapy.

Our People
The Fit4D coaching team is comprised of clinical experts: dietitians, exercise physiologists, nurses, pharmacists, social workers, and personal trainers. Most of our coaches are qualified as Certified Diabetes Educators (CDEs), and have been additionally selected based on their ability to Inspire with Education and Motivate for Behavioral Change. They understand the capability of technology to scale their outreach, while maintaining the optimal mix of personal engagement to create trusted and valued relationships with your patients.

Our Process
Using proprietary, pre-action assessment tools, our coach experts develop a Rapport / Engagement Plan with your patients. This plan involves identifying each customer’s unique Learning Style, Lifestyle, and Medical Condition, and proceeds to optimize an engagement utilizing combinations of high-tech and high-touch technology.
 

Our Plans

The CLINICAL package uses educator engagement to improve the health and wellness of patients with diabetes or other chronic health conditions. Consistent contact with a patient helps to empower them to make positive changes in their lifestyle and motivates them to better self-management and retains them on therapy. The resulting improvement in patient health reduces the direct and indirect healthcare costs and helps avoid the financial impact of critical health events associated with chronic diseases.
